Angela Smith
Optometrist, Owner
Dr. Angie Smith is a native of Stephen, Minnesota and a graduate of the University of North Dakota. In 2006, she received her Doctor of Optometry degree from Pacific University College of Optometry in Forest Grove, OR. During optometry school, Dr. Smith participated in eye care mission trips to Mexico and Africa through the non-profit organization Amigos. These trips gave her the opportunity to work with underprivileged people while expanding her clinical experiences and they continue to influence her work today.
After completing her residency in Washington, she worked in La Crosse, Wisconsin before deciding to move with her family to Crookston in 2009 to work at the Crookston Eye Clinic. In 2012, the Fosston Eye Clinic opened and her patient-care time is spent at both locations.
Dr. Smith holds a membership with both the American and Minnesota Optometric Associations. She has served on boards from Trinity Lutheran Church, Cathedral School and the Crookston Chamber. She and her husband, Nathan, enjoy living in Crookston and stay busy by trying to keep up with their four children: Isabelle, Lucy, Beau and Penelope. When not working, Dr. Smith enjoys reading, preferably on a pontoon or deck. She loves to travel, downhill ski and enjoy the Minnesota summers.
Matthew Forgit
Optometrist, Owner
Dr. Forgit has been a provider at the Crookston Eye Clinic since August of 2004 and at the Fosston Eye Clinic since August of 2012. He grew up on a farm near Fertile, MN. He did his undergraduate work at Concordia College in Moorhead, MN, and attended Pacific University College of Optometry. Clinical rotations at a busy multi-center group practice, a VA hospital, and remote Alaskan villages with the Indian Health Service provided specific experience in ocular disease, ocular trauma, and surgery co-management. Today he enjoys all the variety associated with full-scope primary eye care.

Dr. Forgit is a member of the American and Minnesota Optometric Associations. Aside from eye care, he currently serves on the Villa St Vincent advisory board and Fertile-Beltrami Education Foundation board of directors.

Dr. Forgit and his wife have three children and reside in rural Fertile. When not involved in family or work activities, he enjoys training, hunting, and field-trialing bird dogs.
Kynndyl Giannonatti
Optometrist
Dr. Giannonatti is a native of McIntosh who graduated from Win-E-Mac High School in 2011. She then completed her undergraduate degree at Concordia College in Moorhead, MN and earned her Doctor of Optometry degree from Pacific University College of Optometry in Forest Grove, Oregon. During optometry school, Dr. Giannonatti was a part of a group called Amigos, which provides eyecare to underserved areas across the globe. She traveled with this group on a mission trip to Tanzania, Africa during her third year of school. Her clinical rotations included the Mayo Clinic, Indian Health Services, an urban group private practice, and a VA hospital in Salt Lake City. These internships provided experience in ocular disease, contact lenses, pediatrics, and surgery co-management.
Dr. Giannonatti recently moved to Erskine Minnesota with her husband to work at the Crookston and Fosston Eye Clinics. She looks forward to providing comprehensive eyecare at both locations and becoming involved in the community.
